MyCare is a secure online portal that allows a patient, or person who the patient chooses (proxy), to have secure web-based access to selected medical information and services.
This service is available to Gundersen Health System patients age 18 and over.
MYCARE IS NOT TO BE USED IN AN EMERGENCY. IF YOU HAVE A MEDICAL EMERGENCY, DIAL 911 OR GO TO THE NEAREST EMERGENCY ROOM.
By accessing or using MyCare you agree to be bound by all the terms and conditions of this Agreement. This agreement may be modified, and you agree that such modifications are effective immediately upon posting of the modified version.
MyCare is a service offered to Gundersen Health System patients. You understand that MyCare provides online access to portions of your Gundersen Health System health care record and that you are solely responsible for any sharing of its content that you intentionally or unintentionally communicate to others.
You agree that all communication through MyCare will be in regard to your own health condition(s). Messages you send (and our responses) will be part of your permanent medical record. You understand that asking for advice on behalf of another person could potentially be harmful and shall be considered a violation of these Terms.
It is your obligation to monitor MyCare for responses to your messages and for new messages originating from your doctor's office.
You are asked to provide us with a valid email address when activating a MyCare account. Gundersen Health System will endeavor to notify you via email when new information is available in your MyCare account. This email alert of new information in your MyCare account could be in regard to new test results or other important information about your health care. The email messages will not contain any confidential medical information.
If you change your email address, you should update your email address in MyCare. Only one email address is stored per patient by Gundersen Health System. Your email address may be used for other communication.
You may discontinue your MyCare service by calling 1.800.362.9567, x50303.
Gundersen Health System reserves the right to deactivate your MyCare account at any time, for any reason without notice. Without limiting the forgoing, account deactivation typically will result from: 1) Failure to abide by these Terms and Conditions; or 2) Inappropriate use of messaging (e.g., messaging that is excessive, unnecessary or contains inappropriate content). We will attempt to notify you if your account is deactivated.
Consumer health information is intended to be general in nature and is provided to supplement the care provided by your physician. It is neither intended nor implied to be a substitute for professional medical advice.

Gundersen Health System makes no representations whatsoever about any other Web Site that you may access through MyCare. When you access a non-Gundersen Health System site, you understand that it is independent from Gundersen Health System and that Gundersen Health System has no control over the content on that Web Site.
You are authorizing Gundersen Health System to disclose your health information to you via MyCare. You understand that access to your health information is for your use only, and that you may call your care providerís office for a more detailed explanation of tests results received via MyCare.
